Jack Bloomfield, CAI-II
Jack Bloomfield brings more than 30 years of continuous sobriety and recovery to his skills as an interventionist. In addition, he has worked in the field of addiction and behavioral health serving as a clinical outreach director and coordinator for such well known programs as Caron, The Hanley Center, The Refuge and Delray Recovery Center. During his time in clinical outreach, it offered him the opportunity to get to visit many of the top treatment programs in the country and learn firsthand which facilities he would send his own family members to based on their clinical staff and ethical approaches to treatment. Jack’s work included his personal involvement with families in educating them about addiction and helping them to understand when intervention was needed. In addition, he helped guide them through the process of choosing a treatment center if necessary as well as how best to get the family on board.
As an interventionist, Jack is certified (CAI-II) in the ARISE® model which focuses not only on helping the alcoholic or addict to find recovery but includes the whole family as well. Rather than being a one and done event, the ARISE® model is a minimum six month continuing care process where the immediate family and close loved ones, along with the alcoholic or addict, engage on a path of recovery together.

Those who reach out to us for help are lost, broken and often times at deaths door but we have seen so many who believed that they were hopeless, recover and experience for themselves, the miracle of recovery.”
Jack is married and is the father of two adult children. He is originally from Westchester County, New York and has been living in Florida since 1994.
Rachel Borkon has been on the front lines of working with mental health and addicted individuals and their families for more than twenty years. For the past fifteen years she has been on the recovering side. Rachel majored in Psychology at Rutgers University. She then entered into the addiction field as a behavioral health technician and while helping individuals navigate the early path in recovery, she was also appointed as a group facilitator. Then as a result of working in the field, Rachel was introduced to doing interventions which she immediately felt was a perfect fit and what she felt might be her calling. She came to realize later on that doing this work was not only a calling but her life’s purpose.
Rachel is trained and certified in the ARISE® model of intervention. She was fortunate that in her training, she was under the teaching, guidance and supervision of Dr. Judith Landau, the co-founder of ARISE®.
Rachel is fearless in her approach to helping others and has a way to be able to connect with the most severely troubled that many are not able to. Rachel believes that all of her years as a former addict, coupled with a heightened instinctual sensitivity to others, prepared her for this role. Some people have a gift to be able to reach those who others have dismissed as too far gone… Rachel is one of those people. She is able to be the interruption necessary to the addicted individual as well as be the added support for the entire family.
Alec Rosen, CAICI, NLP, CRC
Adorning the titles of Certified Recovery Coach and ARISE Interventionist Intern, Alec Rosen brings with him a vast amount of experience and expertise to his role at A Design For Living . Born and raised in New York City, Alec discovered by the age of 13 that he would rather receive his education in the real world rather than in a classroom and began working to support his family. However, no stranger to drug and alcohol addiction himself, Alec soon found himself lost in the grips of his own addiction and entered treatment in 2011 and relocated to South Florida.
Upon completing treatment, Alec took a job as a loan officer for several financial institutions and quickly recognized what he was doing was not fulfilling. He decided his true passion was to help others recover as he did. Alec went on to work as Case Manager, Director of Patient Services & Facility Operations, and Admissions Counselor at Wellness Resource Center, a prominent Dual-Diagnosis treatment center in South Florida. Alec went on to attend Bennet Stellar University, where he became a Licensed Master Practitioner of Neuro-Linguistic Programming. (NLP) Alec is trained in the ARISE Intervention Model and fortunate enough to train under co-founder of the ARISE-Network Dr. Judith Landau.
Alec’s thirst for knowledge and deep personal connection to the field of substance abuse, in combination with his strong interpersonal skills help him shine in his current position as Interventionist and Recovery Coach at A Design For Living. He continues to strive to strengthen the community and improve the lives of those in need by delivering exceptional services tailored to meet individual goals and live up to their truest potential.
